29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about December 19, 2013, employees performing framing activities on a commercial construction project were not wearing protective hard hats and exposed to head injuries from flying or falling objects.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about December 19, 2013, employees performing framing activities on a commercial construction project were not protected from falling approximately 10 feet to the ground below by personal fall protection or other fall protection systems.
