29 CFR 1926.651(f): A warning system such as barricades, hand or mechanical signals, or stop logs was not utilized when mobile equipment was operated adjacent to an excavation, when such equipment was required to approach the edge of an excavation: 189 Attucks Lane, an equipment operator backed up and fell into the excavation with the equipment seriously injuring an employee from a different company. The operator failed to use any warning system to alert him to the edge of the excavation when his view was obstructed.
