29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:    a)  Corbin Heights Project - On or about 1-10-14 - employees were observed using a four-way metal receptacle box with cord and plug as an extension cord run on the floor of the work area.  The metal box had knockouts and was designed to be wall mounted.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:    a)  Corbin Heights Project - On or about 1-10-14 - Employees were observed using a job made extension cord system that did not have the plug properly attached to the wiring creating no strain relief at the plug.
