29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides,    a)  #1 Ladle Metallurgical Furnace; On or about January 14, 2014 and at times prior thereto the employer exposed employees to fall and burn hazards in that employees were allowed to walk and work on a platform with an open side over the furnace and the next level below.     b)  #2  Ladle Metallurgical Furnace; On or about January 14, 2014 and at times prior thereto the employer exposed employees to fall and burn hazards in that employees were allowed to walk and work on a platform with an open side over the furnace and the next level below.    c)  Ladle Refractory Area; On or about January 14, 2014 and at times prior thereto the employer exposed employees to fall hazards in that employees were allowed to walk and work on a platform with an open side where the next lower level was 7 foot 5 inches.    d)  Tundish Area; On or about January 14, 2014 and at times prior thereto the employer exposed employees to fall hazards in that employees were allowed to walk and work on a platform with an open side where the next lower level was 8 foot.

29 CFR 1910.305(b)(2): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:  a)  Rolling Mill; On or about January 14, 2014 and at times prior thereto the employer exposed employees to electrical hazards in that employees were allowed to walk and work around several LB's - conduit bodies that did not have covers.
