29 CFR 1926.651(c)(2): Means of egress from trench excavations. A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. (a) North Jobsite - On or about January 14, 2014 an employee working in a 95 feet long x 5 feet 06 inch deep x 5 feet wide vertical side trench did not have a means of egress.

Employees exposed to public vehicular traffic shall be provided with, and shall wear, warning vests or other suitable garments marked with or made of reflectorized or high-visibility material. (a) Jobsite - On or about January 14, 2014 employee did not wear a reflective vest while working near vehicular traffic.

29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section except when: (a) Jobsite - On or about January 14, 2014 employee working in an excavation that was greater than 5 feet deep. Employee working in type B soil was not protected from cave-in hazard by a trench shield protective system or sloping and benching system.
