29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:    Employees mixing drilling mud using caustic soda at Rig #15 located at SEC 24, TWP 34, R 4, 2440 fnl, 880 fel, Caldwell, Kansas using caustic soda were exposed to respiratory hazards in that a medical evaluation was not performed prior to using the N95 filtering face-pieces.

29 CFR 1910.134(f)(1): The employer did not ensure that employee(s) required to use a tight-fitting facepiece respirator passed the appropriate qualitative fit test (QLFT) or quantitative fit test (QNFT):    Employees mixing drilling mud using caustic soda at Rig #15 located at SEC 24, TWP 34, R 4, 2440 fnl, 880 fel, Caldwell, Kansas using caustic soda were exposed to respiratory hazards in that fit tests where not done for each employee required to wear the N95 filtering face-pieces.

29 CFR 1910.151(b): There was neither an infirmary, clinic, or hospital used for the treatment of all injured employees in near proximity to the workplace nor a person or persons adequately trained to render first aid:    Employees working on Rig #15 located at SEC 24, TWP 34, R 4, 2440 fnl, 880 fel, Caldwell, Kansas were exposed to hazards associated with delayed medical care.  The employer did not train employees in cardiopulmonary resuscitation and first aid.
