29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:  (a) The employer had not established or implemented those elements of a written respiratory protection program to ensure than an employee using an elastomeric cartridge respirator on a voluntary basis during spray painting and sweeping inside of containers was medically able to use the respirator and that the respirator was cleaned, stored, and maintained so that it did not present a health hazard to the user. 29 CFR 1917.24(a): Employees were not removed from an enclosed space when the carbon monoxide concentration exceeded a ceiling of 100 parts per million (ppm) (0.01%).  (a) A mechanic who operated Work Truck No. 100 was determined to be exposed to carbon monoxide at a concentration ranging between 102 and 156 ppm when entering the back of the truck while a gasoline-powered electric generator and/or air compressor were running.
