29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:  (a) On or about January 22, 2014, and at times prior, the employer did not establish a respiratory protection program which includes a required medical evaluation, for an employee who voluntarily wore a half-face respirator.  Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.1200(f)(5)(i): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the identity of the hazardous chemical(s) contained therein:  (a)  On or about January 22, 2014, and at times prior, the employer did not ensure employees were protected from chemicals in bottles that were not properly labeled such as blue fluid that was in a bottle identified as "Alcohol" in the detail area of the service shop, and pink fluid in a bottle identified as "Interior clean" near lift 8 in the service shop.  Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.1200(f)(5)(ii): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the appropriate hazard warnings:  (a)  On or about January 22, 2014, and at times prior, the employer did not ensure employees were protected from chemicals in bottles that were not properly labeled in that a bottle with blue fluid and a bottle with pink fluid did not have hazard warnings identifying physical and health hazards of their contents.
