The guardrail system was not equipped with a midrail.  2. No guardrail system, fall arrest system or other equivalent means had been in place.  On or about 1/20/14, an employee preparing for hvac work and carrying debris from the work area, was exposed to the hazard of falling 8 feet 10 inches to the lower level without the use of guardrails, fall arrest systems or other equivalent means.  The employee fell through the unguarded floor opening and sustained a broken bone to his arm.
