29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:      (a) Location: 4 Latti Farm Rd, Millbury, MA: On or about January 30, 2014 two employees were observed working inside an aerial lift approximately 15 feet above ground level without being attached to the boom or basket as required by this subpart.    Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:   (a) Location: 4 Latti Farm Rd, Millbury MA: On or about January 30, 2014 an employee working for McGovern Custom Homes operating an aerial lift while performing work on an exterior commercial building had not received training from an authorized person to operate an aerial lift.
