29 CFR 1910.147(d)(5)(ii): Where there was a possibility of re-accumulation of stored energy to a hazardous level, verification of isolation was not continued until the servicing or maintenance was completed, or until the possibility of such accumulation:    On or about January 29, 2014, NTS Incorporated of Texas exposed employees in the process of changing valves on an existing pipeline to fire/explosion hazards.  The employer did not ensure that verification of isolation of hydrocarbons and hydrocarbon vapors was continued until the maintenance operation was completed.

29 CFR 1910.147(f)(3)(i): When maintenance was performed by a crew, craft, department or other group, a procedure was not utilized that afforded the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device:      On or about January 29, 2014, the employer did not utilize a procedure to lockout the section of pipe being worked on that afforded the employees protection against the release of hazardous energy afforded by an individual lockout.
