29 CFR 1910.253(a)(2): Acetylene used in an oxygen-fuel gas system was generated, piped or utilized at a pressure in excess of 15 psig (103 kPa gauge pressure) or 30 psia (206 kPa absolute). (a) Shore side of Black Hawk Tug: Employee was using oxygen-fuel gas for welding with the acetylene gauge registering 18 psig. This exposed employees to a potential explosive/fire hazard from using acetylene at pressures above 15 psi.

29 CFR 1915.504(b)(3): The employer did not post a fire watch when there was combustible material less than 35 feet in either the horizontal or vertical direction that could not be removed, was not protected with flame-proof covers or shielded with metal fire-resistant guards or curtains. (a) M/V Chinook engine space: Employees were performing hot work to include welding and grinding with combustibles within 35 feet. This exposed employees to smoke inhalation hazards from combustibles igniting within 35 feet of hotwork.

29 CFR 1915.1026(d)(2)(iv): The employer used the scheduled monitoring option, and monitoring revealed employee exposures to chromium (VI) to be above the permissible exposure limit; however, the employer did not perform periodic monitoring at least every three months. (a) SOMAR Shipyard: Results from a third party industrial hygiene survey for hexavalent chromium CR(VI) had a result of 45 micrograms for personal sampling and 21 micrograms for area sampling. The employer did not perform any additional monitoring after this survey to establish objective data to ensure engineering controls were adequate. This exposed employees to potential allergic contact dermatitis and long term exposure could result in an increased risk of lung cancer.
