29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:    On February 3, 2014, an employee working from a residential roof, twenty one (21) feet two (2) inches above the ground, was tied to an anchor point below the working surface, exposing the employee to the hazards associated with a free fall more than 6 feet.     There is no abatement certification or documentation required for this item.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about February 3, 2014, an employee performing residential construction operations on the roof of a residential property, twenty one (21) feet two (2) inches above the ground, was not protected from falling, thereby exposing the employee to fall hazards.    Horizon Carpentry, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 910473, citation number 2, item number 1 and was affirmed as a final order on July 23, 2013, with respect to a workplace located at 9685 Cummings, Huntley, IL 60142.    There is no abatement certification or documentation required for this item.
