29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  a) On or about February 6, 2014, in the workplace:  The employer had not conducted an annual inspection of the energy control procedures in use in the workplace.  Lockout procedures of the employer are performed on equipment such as electric tile saws, the air operated gang nail press, the overhead hoist system, and a twelve inch table saw.    This item was corrected during the inspection.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial trucks were performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly.  a)      On or about February 6, 2014, in the workplace:  the employer did not obtain new data plates for Toyota powered industrial trucks when they were utilized with grated attachments to push and pull modular homes weighing up to approximately 40,000 pounds.  Note: Abatement certification is required for this item.

29 CFR 1910.303(f)(1): Each disconnecting means required by this subpart for motors and appliances was not legibly marked to indicate its purpose, unless located and arranged so the purpose is evident.  a) On or about February 6, 2014, in the workplace:  The disconnecting means for Transformer D-2, rated at 480 Volts, was not labeled to indicate its purpose.  This item was corrected during the inspection.
