29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws.  a)  On February 6, 2014, at the Father Duenas Memorial School Chapel and Tower project, the yellow with black stripe extension cord used to extend power to the hand tools did not have strain relief, and its insulation was separated from the outer sheathing.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a)  At Father Duenas Memorial School Chapel and Tower Project dome area, employees applying sealant did not have fall protection and were exposed to a fall hazard of more than 20 feet.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder must not be used as a step.  a)  At Father Duenas Memorial School Chapel and Tower Project dome area, the employee used the top step of a step ladder as a step in applying sealant.  The employee was exposed to a potential fall hazards of more than 20 feet to the ground.
