29 CFR 1926.403(b)(1)(i): Safety of equipment was not determined on the basis of suitability of the equipment for an identified purpose as evidenced by listing, labeling, or certification for that identified purpose.      Instance 1. On February 13, 2013 an employee was exposed to the hazards associated with electrical shock while using a relocatable power tap that was not listed and labeled for outdoor use.        Instance 2. On February 13, 2013 an employee was exposed to the hazards associated with electrical shock while using a 4"x4" knockout junction box as a receptacle box.  The Junction box was not listed and labeled to be used as a free standing junction box.      Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
