29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as follows: West and south side of the residential construction. On or about February 13, 2014, employees were exposed to a fall of up to 20 feet in elevation to the ground level below, from a scaffold that only had 2 planks for the decking where there was room for 3 planks, while preparing windows for stucco application. Guillermo Lopez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(b)(1), which was contained in OSHA inspection number 894295, citation number 01, item number 001, and was affirmed as a final order on 05/16/2013, with respect to a workplace located at 24045 Stately Oaks, San Antonio, TX 78260.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used. Crossbraces were used as a means of access: West side and south side of the residential construction. On or about February 13, 2014, employees were exposed to a fall of greater than six feet in elevation to the ground level below, from a scaffold that had no ladder for access, while preparing window units for stucco application. Guillermo Lopez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(e)(1), which was contained in OSHA inspection number 894295, citation number 01, item number 003, and was affirmed as a final order on 05/16/2013, with respect to a workplace located at 24045 Stately Oaks, San Antonio, TX 78260.

On or about February 13, 2014, employees were exposed to a fall of greater than ten feet in elevation to the ground below from a scaffold that lacked crossbracing on the inside of the scaffold, while preparing window units for stucco application.
