29 CFR 1926.850(a): An engineering survey was not performed by a competent person to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations.(a) On or about February 12, 2014, a written engineering survey of the structure was not performed prior to employees engaging in demolition operations.

29 CFR 1926.859(g): During demolition, continuing inspections by a competent person shall be made as the work progresses to detect hazards resulting from weakened or deteriorated floors, or walls, or loosened material. No employee shall be permitted to work where such hazards exist until they are corrected by shoring, bracing, or other effective means. On or about February 12, 2014, during demolition operations, a competent person did not perform inspections of the structure to detect hazards such as but not limited to: exposure to un-braced / un-shored flooring.

29 CFR 1926.856(a): Mechanical equipment shall not be used on floors or working surfaces unless such floors or surfaces are of sufficient strength to support the imposed load. On or about February 12, 2014 a structural collapse occurred when a Caterpillar 345C (serial number PJW00642) Backhoe was operated over a floor that could not support the load.
