29 CFR 1926.102(a)(1):  Eye and face protective equipment was not provided when machines or operations presented potential eye or face injury:      (a)  Mayhem Restoration & Remodel at 4315 Cook St, Denver, CO 80216:  On February 12, 2014 the exposing employer did not ensure employees were provided eye protection when operating pneumatic nail guns while installing roofing material.  This condition exposed employees to the hazard of projectiles to the eyes.    (b)  Mayhem Restoration & Remodel at 4315 Cook St, Denver, CO 80216:  On February 12, 2014 the exposing employer did not ensure employees were provided eye protection when removing roofing material.  This condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system:      (a)  Mayhem Restoration & Remodel at 4315 Cook St, Denver, CO 80216:  On February 12, 2014 the exposing employer did not ensure that employees were protected from injury, in that employees removing and installing shingles on a single-story residential building were not utilizing a fall protection system.  The pitch of the roof was 4.5-in-12.  This condition exposed employees to falls ranging from 15 to 9 feet.
