29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. Throughout facility - On or about February 14, 2014 employees (full-time / part-time / temporary) were required to maintain and/or repair equipment including, but not limited to: A) Resaw / Division St Alley / McDonough / 54 54-1365 B) Resaw / Main Mill / Stenner VHM36 / U5302 C) Ripsaw / Main Mill / XL Ripper 104 / 767404 D) Crosscut saw / Main Mill / DeWalt GF 65 / 125738 E) Crosscut saw / East Saw Room / DeWalt GF 65 / 39655 F) Unit saw / Front yard / Pac-Trail 116-TMPA / 200505 The employer did not establish an energy control program which includes energy control procedures, employee training, and periodic inspections, thereby exposing employees to the hazards associated with the unexpected start up and/or re-energization of equipment.

At least two exit routes must be available in a workplace to permit prompt evacuation of employees and other building occupants during an emergency, except as allowed in paragraph (b)(3) of this section. The exit routes must be located as far away as practical from each other so that if one exit route is blocked by fire or smoke, employees can evacuate using the second exit route. The employer was not able to provide documentation that employees engaged in the operations of powered industrial vehicles had been certified in accordance with OSHA standards.
