OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard from standing on the top step of an eight foot fiberglass portable step ladder:  a) Paint storage area roof, main shop area, 322 Lavansville Road, Somerset, PA 15501:  Observed on or about February 20, 2014, employees were exposed to a fall hazard of approximately 8 feet 8 inches to the concrete floor below while using the top step of an 8 foot portable fiberglass step ladder to access the roof of the paint storage area.  Employees accessed the paint storage roof area on a regular basis to change the paint booth light bulbs, retrieve paint booth filters, and retrieve boxes of supplies.

Employees accessed the paint storage roof area on a regular basis to install paint booth lights, get paint booth filters, and to retrieve boxes of materials.  The employees were exposed to a fall hazard of approximately 8 feet 8 inches to the concrete floor below.

Two buttons on the pendant control were missing covers and other buttons had worn off labels.  The lack of markings on each of the pendant control buttons exposed employees to crushed-by hazards while using the overhead crane to move truck bodies and truck body components.     b)  Main Bay, N.E., 322 Lavansville Road, Somerset, Pennsylvania 15501:  Observed on or about February 24, 2014, the pendant controls on a Demag 5 ton overhead crane (serial number S08284) were not clearly marked.  The covers on two of the control buttons were missing.
