29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about 02/20/2014 at the 42 Beach Avenue reroofing project, employees were ripping shingles from a roof deck and installing plywood and new shingles to the roof deck and no fall protection system was in use.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about 2/20/2014 at the 42 Beach Avenue reroofing project, employees including the employer were using an extension ladder to access and descend to and from the roof deck and the ladder did not extend 3 feet past the landing.

29 CFR 1926.1053(b)(21): Employees were not grasping the ladder with at least one hand when progressing up and/or down the ladder:  On or about 02/20/2014 at the 42 Beach Avenue reroofing project, an employee was carrying shingles and plywood on his left shoulder while climbing up an extension ladder and the employee was using one hand to leap from rung to rung while climbing the ladder.

29 CFR 1926.1053(b)(22): An employee shall not carry any object or load that could cause the employee to lose balance and fall:  On or about 02/20/2014 at the 42 Beach Avenue reroofing project, an employee was carrying 4 feet by 8 feet by 7/16 inch OSB boards while climbing up a ladder.  The employee was also carrying bundles of shingles while climbing a ladder.
