Deficiencies noted during the inspection included, but were not limited to, not taking a verification step after locking equipment out, each employee working on equipment was not putting a lock on energy isolating devices, failing to lock out machines prior to service/maintenance work, and not performing a physical energy control action on equipment/machinery on an annual basis to ensure that it worked properly.
