29 CFR 1910.242(a): Hand and portable powered tools or equipment were not kept in safe condition:       a) Jobsite, 656 New Castle Road, Slippery Rock, PA 16057:  On or about June 9, 2016, a Stihl TS400 brand gas powered cut-off machine was being used with a blade type that was prohibited by the manufacturer of the cut-off machine.  The cut-off machine was being used with an A.G.E. brand demolition rescue blade (tool number DB12-120C).  The Owner's Manual for the cut-off machine prohibits the use of rescue blades or toothed blades of any nature because the use of such blades could generate substantially greater reactive forces.
