29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:    On or about February 26, 2014, and at times prior thereto, employees were working on a rig site, Bison Drilling Rig #80. The trailer that housed the top drive mechanics was not grounded. Employees were exposed to potential electric shock or burns and/or electrocution.    Light Tower Rentals, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.304(g)(5), which was contained in OSHA inspection number 825042, citation number 1, item number 1 and was affirmed as a final order on April 30, 2013, with respect to a workplace located 9 miles southeast of Pecos, TX.

29 CFR 1910.305(g)(1)(iv)(C): Flexible cords and cables were run through dorways, windows or floors:  On or about February 26, 2014, employees were working on Bison Drilling Rig #80. A yellow 50 foot electrical cord was coming out of the window of the top drive mechanics' trailer and was being pinched.  The cord was used to power the sewage pump. Employees were exposed to potential electric shock and/or burn hazards.
