29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   Location: 15 Cottage Street, Easthampton MA. On or about 3/03/2013, employees were using a Louisville fiberglass 8 ft. ladder to gain access to the roof and the ladder side rails did not extend at 3 feet above the landing surface as required by this subpart.

29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems:     Location: 15 Cottage Street, Easthampton MA. On or about 3/03/2013, an employee was observed lowering a rope from the roof top to hoist equipment was exposed to a fall greater than 36 feet above the ground below without the use of any fall protection.   The Patriots Environmental Corporation was previously cited for a violation of this occupational safety and health standard, 29CFR 501(b)(13) which was contained in OSHA inspection number 312344591, citation number 1, item number 1 and was affirmed as a final order on 12/01/2010, with respect to a workplace located at Bldg. 305 Meyecord Ave & Peary Street, Naval Base Newport, RI.
