29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a) Zemco Tool & Die, 113 South West Street, Williamstown, Pennsylvania - Employees performed servicing and maintenance activities, such as die-changing, mold changing, and installing new motors, on a variety of machinery and equipment, including, but not limited to, the Bruderer AG Typ BSTA 20 four-post high-speed mechanical stamping press and the Toyo Machinery & Metal Co. Ltd. Plastar T1-90GS plastic injection molding machine, and the employer did not develop and implement an energy control program, consisting of machine specific energy control procedures, group lockout/tagout, testing and positioning procedures, periodic inspections, and employee training, exposing employees to hazards of getting caught-in and struck-by moving machine parts, on or about November 1, 2013.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a) Zemco Tool & Die, 113 South West Street, Williamstown, Pennsylvania - Employees performed servicing and maintenance activities, such as die-changing, mold changing, and installing new motors, on a variety of machinery and equipment, including, but not limited to, the Bruderer AG Typ BSTA 20 four-post high-speed mechanical stamping press and the Toyo Machinery & Metal Co. Ltd. 29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    (a) Press Room - A homemade extension cord, consisting of a metal knockout box with two receptacles attached to a flexible cord, which supplied power to the tool safety and the de-reeler for the Bruderer AG Typ BSTA 20 four-post high-speed mechanical stamping press, was not installed in accordance with the box's listing, labelling, or certification in that the knockout box was not securely and rigidly mounted to the building or other support structure, exposing employees to injurious contact with electrical current, on or about January 2, 2014.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
