29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a) On or about 03/07/2014, a laborer employee was not wearing a faceshield to protect his face from flying particles, sparks and potential kickback while using a handheld chop saw to cut a bundle of metal rebar.
