Merritt Roofing & Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 931309, citation number 01, item number 001 and was affirmed as a final order on August 19, 2013, with respect to a workplace located at 13707 Hunterwood Road, Jacksonville, Florida 32225  29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels are not protected by guardrail systems, safety net system, or personal fall arrest system, nor are employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a. On or about March 07, 2014, on the 8:12 pitch roof above the garage, employees performing roofing operations are not protected from an 8 feet fall hazard from the eave by the use of a fall protection system. On or about March 07, 2014, on the 8:12 pitch roof and on the left side of the garage, employees performing roofing operations are not protected from a 10 feet fall hazard from the eave by the use of a fall protection system. The employer does not protect each employee on their jobsites by ensuring employees are wearing a personal fall arrest system or by using appropriate guardrail or safety net system. This violatioinb was most recently observed as a job site located at 2636 Cortez Road, Jacksonville, FL.  c. On or about March 07, 2014, on the 8:12 pitch roof, employees performing roofing operations are not protected from a 15 feet fall hazard from the gable to the ground by the use of a fall protection system. The employer does not protect each employee on their jobsites by ensuring employees are wearing a personal fall arrest system or by using appropriate guardrail or safety net system. This violation was most recently observed as a job site located a 2636 Cortez Road, Jacksonville, FL.  Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance, including an explanation of how these steps protect its employees from fall hazards.
