In completed installations, each outlet box did not have a cover, faceplate, or fixture canopy.   On 2/25/2014, Shelby County Motors, Inc. had not installed covers on various wall outlets and a circuit breaker junction box located in the ferrous metal area.

On 2/25/2014, Shelby County Motors, Inc. had not ensured cords had proper strain relief in the following areas:  a.  Two switches on the conveyor in the nonferrous metal area. b.  Junction box/circuit breaker panel in the nonferrous metal area. c.  Control panel for the baler in the nonferrous metal area.
