29 CFR 1910.269(l)(1)(i): Except as provided in paragraph (l)(1)(ii) of this section, at least two employees shall be present while the following types of work are being performed:  a. On January 18, 2014, at the Cabras 1 & 2 Power Plant, an employee was repairing equipment near energized electrical equipment by himself, exposing himself to an electrical hazard.

29 CFR 1910.269(l)(2): The employer did not ensure that no employee(s) approached or took conductive objects closer to exposed energized parts than set forth in Tables R-6 through R-10:  a. On January 18, 2014, at the Cabras 1 & 2 Power Plant, an employee climbed the transformer to work on electrical equipment within close proximity of energized parts, exposing himself to an electrical hazard.
