29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: a. West roof area - the platform on the Genie High Reach model GTH-844 S/N GTH0807A-11657 telehandler used to transport employees off the roof was unsecured sheets of plywood resting over forks. Violation observed on or about 3/11/14.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a. West roof area - during decking operations, fall protection was not used on a 6:12 pitched roof and exposed employees up to a 26 foot fall hazard. Violation observed on or about 3/11/14.
