29 CFR 1926.602(a)(9)(ii): Earth moving or compacting equipment which had an obstructed view to the rear was operated in reverse gear; such equipment did not have in operation a reverse signal alarm distinguishable from the surrounding noise level or did an employee signalled that it is safe to operate in reverse gear: a) Paterson Avenue and Rose Street, Wallington, NJ: Employees were exposed to struck by hazards while working with a Komatsu PC95 excavator that did not have a working back-up alarm, on or about 3/12/2014. John Garcia Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.602.(a)(9)(ii), which was contained in OSHA inspection number 314682147, citation number 01, item number 001 and was affirmed as a final order on 5/23/2012, with respect to a workplace located at Pulaski Avenue, Wallington, NJ.
