29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as grabrail, was not provided to assist employees in mounting and dismounting the ladder.   (a) Building 17, East Side - On or about March 11, 2014 one employee was exposed to fall hazard when ascending and descending a ladder that was not extended at least 36 inches above the upper landing surface nor was ladder secured at its top with a grasping device.
