29 CFR 1910.304(g)(5): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective. a.) Ceres Marine Terminal, Shop area: On or about 03/07/14, the path to ground to the workbench receptacles were not permanent and continuous, in that, the internal ground wire to the male attachment plug was never attached to the ground prong, thereby exposed employees to a potential shock hazard.

29 CFR 1918.35: Open weather deck hatches around which employees must work that are not protected to a height of 24 inches (.61 m) by coamings shall be guarded by taut lines or barricades at a height of 36 to 42 inches (.91 to 1.07 m) above the deck, except on the side on which cargo is being worked. Any portable stanchions or uprights used shall be supported or secured to prevent accidental dislodgement. a.) Virginia International Terminal, Vessel Tokyo Express, Hatch #46: On or about 03/14/14, sections of an open hatch were not provided with barricades or taut lines thereby exposed employees to a 52' fall.
