29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times.    (a)  Spy Island, Mud plant, south main door and compressor area:  On or about 17 March 2014 and times prior thereto; two instances were noted where the emergency exit lights were inoperative.    (b) Spy Island, Muster Bldg, Logistics area:  On or about 17 March 2014 and times prior thereto; two instances were noted where the emergency exit lights were inoperative.    This condition exposed employees to delays in exiting the building and smoke inhalation, fire, and tripping hazards.

This condition exposed employees to an electrical shock and burn hazard.

29 CFR 1910.305(g)(1)(iv)(D): Flexible cords and cables were attached to building surfaces.    Spy Island, Muster Bldg:  On or about 17 March 2014 and times prior thereto, a blue flexible cord, approximately 25-feet in length was permanently affixed to an storage racks and routed into KNAACK Model 112 Job Master cabinet.  This condition exposed employees to an electrical shock hazard.
