OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees.   Muster building:  On or about 17 March 2014 and times prior thereto; warehouse racks were not secured to the wall or floor.  Storage racks did not have the maximum safe load limits posted.  This condition exposed employees to a struck by hazard.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch.     Welding shop,  on or about 17 March 2014 and times prior thereto; the work rest on a Baldor bench grinder, Serial Number F185 was not properly adjusted to a maximum of 1/8 of an inch from the abrasive wheel. The gap between the abrasive wheel and work rest measured approximately 3/8 of an inch. This condition exposed employees to a caught in hazard.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch.       Welding shop,  on or about 17 March 2014 and times prior thereto; the tongue guard on a Baldor bench grinder, Serial Number F185 was not properly adjusted to a maximum of 1/4 of an inch from the abrasive wheel. The gap between the abrasive wheel and tongue guard measured approximately 1/2 of an inch. This condition exposed employees to a caught in hazard.

29 CFR 1910.304(f)(1)(iv): Overcurrent devices for circuits rated 600 volts, nominal, or less, were not readily accessible to each employee or authorized building management personnel.     Boiler room, on or about 17 March 2014 and times prior thereto; the employer did not ensure that the electrical panel was readily accessible in the event of an electrical emergency. A trash can was stored in front of a 600 volt electrical panel labelled 159 impeding access. This condition exposed employees to an electrical shock hazard.

29 CFR 1910.1200(f)(5): Chemical manufacturers, importers, or distributors did not ensure that each container of hazardous chemicals leaving the workplace was labeled, tagged, or marked in accordance with 29 CFR 1910.1200.  Boiler room; on or about 17 March 2014 and times prior thereto; an unmarked plastic spray bottle, containing an unknown chemical substance was not marked identifying the product and the hazard warnings associated with it. This condition exposed employees to chemical burns, poisoning, and skin dermatitis.
