29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows:    WORKSITE: The peak bracket attachment devices, used by employees engaged in roofing work approximately 12 feet above the ground were not installed and used according to the manufacturers installation and use instructions.

29 CFR 1926.502(d)(17): The attachment point of the body belt shall be located in the center of the wearer's back. The attachment point of the body harness shall be located in the center of the wearer's back near shoulder level, or above the wearer's head.  WORKSITE: The employees, engaged in roofing work on an 11/12 steep pitch roof approximately 12 feet above the ground, had the lifelines attached to the front side of the personal fall arrest systems.

29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.    WOKRSITE: The employees performing roofing work, using pneumatic nailing devices were not wearing any eye protection.       Guallpa Contractors LLC was previously cited for a violation of this occupational safety and health standard (29 CFR1926.102(a)(1)), which was contained in OSHA inspection number 98472, citation number 1, item number 1, affirmed as a final order on July 9, 2013, with respect to a workplace located at 913 E. Broadway, Stratford, CT 06615.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.     WORKSITE: The employees, engaged in roofing activities on an 11/12 steep pitched roof approximately 12 feet above the ground, were not protected against the hazard of falling by the use of appropriate fall protective devices/systems.    Guallpa Contractors LLC was previously cited for a violation of this occupational safety and health standard (29 CFR1926.501(b)(13)), which was contained in OSHA inspection number 98472, citation number 1, item number 2a, affirmed as a final order on July 9, 2013, with respect to a workplace located at 913 E. Broadway, Stratford, CT 06615.
