29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  Worksite: The employee who was installing plywood roof sheathing with a pneumatic nail gun was not wearing eye protection.   Guallpa Contractors, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.102(a)(1) which was contained in OSHA inspection number 908472, citation number 1, item number 1, issued on 6/10/2013 with respect to a workplace located at 913 E. Broadway, Stratford, CT and became a final order on 7/11/2013.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b).  Worksite: The employees who were working from the residential roof at a height above six feet were not protected from fall hazards by the use of guardrails, safety nets or personal fall arrest systems.   Guallpa Contractors, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 908472, citation number 1, item number 2a, issued on 6/10/2013 with respect to a workplace located at 913 E. Broadway, Stratford, CT and became a final order on 7/11/2013.
