29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) On or about 03/25/14, on jobsite at 2-story residential house at 78 Gardenwood Ln., Tonawanda, NY: Four employees engaged in construction work were not trained to recognize potential hazards on this type of jobsite including but not limited to falls from roofs, ladders, skylights, stairs & scaffolds, electrical hazards, unguarded tools/machines, fire hazards, and the need for personal protective equipment to protect them from various hazards. They also were not trained in the applicable OSHA regulations to control or eliminate these hazards. ABATEMENT CERTIFICATION REQUIRED

ABATEMENT CERTIFICATION REQUIRED

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) On or about 03/25/14, on the main roof of a 2-story house at 78 Gardenwood Ln., Tonawanda, NY: Four employees engaged in roofing work were exposed to a potential fall of approx. 18 ft. because no fall protection system was utilized. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about 03/25/14, on the front side of a 2-story residential house at 78 Gardenwood Ln., Tonawanda, NY: Four employees engaged in roofing work were exposed to a potential fall of approx. 18 ft. to the ground when an extension ladder was used to access the roof from the ground. The siderails of the ladder only extended approx. 16 inches above the roof edge. b) On or about 03/25/14, on the Back side of a 2-story residential house at 78 Gardenwood Ln., Tonawanda, NY: Employees engaged in roofing work were exposed to a potential fall of approx. 11 ft. to the ground when an extension ladder was used to access the roof from the ground. The siderails of the ladder only extended approx. 18 inches above the roof edge. NO ABATEMENT CERTIFICATION REQUIRED
