29 CFR 1910.95(b)(1): When employees were subjected to sound exceeding those listed in Table G-16, feasible administrative or engineering controls were not utilized: On April 3, 2014, a temporary employee operating the extruder was exposed to noise at an 8-hour time weighted average of 94.19 dBA, approximately 1.05 times the calculated permissible exposure limit of 90 dBA. The sample was collected for 479 minutes; exposure calculations include a zero increment for the 1 minute not sampled. Abatement documentation required per 29 CFR 1903.19(d).

29 CFR 1910.95(k)(1): The employer did not train each employee who is exposed to noise at or above an 8-hour time-weighted average of 85 decibels in accordance with the requirements of 29 CFR 1910.95(k). The employer did not institute a training program and ensure employee participation in the program: On or about March 28, 2014, the employer had not provided hearing conservation training to employees exposed to noise in excess of the action level of 85 decibels for an 8 hour time weighted average. On April 4, 2014, the highest noise level found in the facility was approximately 94.19 decibels for an 8 hour time weighted average. Abatement documentation required per 29 CFR 1903.19(d).
