29 CFR 1926.416(a)(1): The employer permitted an employee to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work, and the employee was not protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means:    a) Jobsite, 626 Upper Tabernacle Road, Donegal, PA: On or about March 27, 2014, during roofing activities, the employer did not ensure that employees were protected from contact with an overhead power line in that, during laddervator raising/lowering activities, the laddervator made contact with an overhead power lines, causing serious injuries to at least two employees.

The employer shall ensure that electrical equipment was free from recognized hazards that are likely to cause death or serious physical harm to employees:     a) Jobsite, 626 Upper Tabernacle Road, Donegal, PA: On or about March 27, 2014, during roofing activities, the employer did not ensure that the power cord for the laddervator motor device in good repair in that the outer jacket was torn/damaged, with duct tape used over some areas.
