29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:     a) 101-104 Kelly Drive, Edison, NJ:  Employees were observed utilizing pneumatic Hitachi nailers and Dewalt circular saws during roofing operations without the use of eye or face protection.     Condition observed on/or about March 31, 2014.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented:       a) 101-104 Kelly Drive, Edison, NJ: Employees were observed performing roofing operations at a height of approximately 20' to the ground below with no fall protection in place, exposing them to fall hazards.      Condition observed on or about March 31, 2014.     LSS Home Improvement, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number644438, citation number 1, item number 1 and was affirmed as a final order on 02/12/2013, with respect to a workplace located at 1118 - 1129 Brian's Way, Wayne, PA.
