29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:     a.  At the Car Wash and Marine Bank, off of 6th street in Springfield, IL, employee changing billboards were exposed to a fall hazard while using a cable (horizontal life line) on the platform(s) because the fall protection system was not designed, install and used to provide adequate fall protection which maintains a safety factor of at least two.    b.  At the Kincaid, IL location employees installing billboards were exposed to a fall hazard of approximately 15 ft. 7in.  Employees working in between the signs were unhooking the lanyards from the cable, and climbing the panels in order to secure the sign.
