29 CFR 1910.252(b)(2)(iii): Protection from arc welding rays. Where the work permits, the welder should be enclosed in an individual booth painted with a finish of low reflectivity such as zinc oxide (an important factor for absorbing ultraviolet radiations) and lamp black, or shall be enclosed with noncombustible screens similarly painted. Booths and screens shall permit circulation of air at floor level. Workers or other persons adjacent to the welding areas shall be protected from the rays by noncombustible or flameproof screens or shields or shall be required to wear appropriate goggles.    a. Hallways Between Welding Areas/Tower International - On or about April 3, 2014, the employer had not ensure that employees required to walk on hallways and around the numerous welding areas/sections were protected from arc welding rays.  Employees were thereby exposed to radiation hazards. The locations were the following;     1. At the South end of the Front Structure - Pony Line - pedestrians around the left and right hand stations were not protected from arc welding rays (manual spot welding).    2. At the Middle Section of the Front Structure - Pony Line - pedestrians were not protected from arc welding rays (automatic robotic welding).      3. At the Front Structure - Main Line - pedestrians around the right hand station were not protected from arc welding rays (manual spot welding).

29 CFR 1910.252(c)(1)(ii): Screens. When welding must be performed in a space entirely screened on all sides, the screens shall be so arranged that no serious restriction of ventilation exists. It is desirable to have the screens so mounted that they are about 2 feet (0.61 m) above the floor unless the work is performed at so low a level that the screen must be extended nearer to the floor to protect nearby workers from the glare of welding.   a. Welding Areas/Tower International - On or about April 3, 2014,welding operations were being done in areas that were not totally enclosed on all sides with welding curtains or screens. Employees were thereby exposed to radiation hazards. The locations and conditions found were the following;    1. On the left hand (LH) of the front structure (Pony Line), the manual welding area had only one (1) portable screen.    2. On the right hand (RH) of the front structure (Pony Line), the manual welding area had only one (1) portable screen.    3. On the middle of the Tote Isle (LH)/Main Alley Way, the robotic welding section was not totally enclosed on all sides.    4. On the middle of the Tote isle (RH)/Main Alley Way,  the robotic welding section was not totally enclosed on all sides.   5. On the right hand (RH) of the front structure (Main Line), the manual welding area had only one (1) portable screen.

29 CFR 1910.132(e): Defective and damaged equipment. Defective or damaged personal protective equipment shall not be used.   a. Front Structures Welding Areas/Main Alley and Pony Lines - On or about April 3, 2014, welder operators were wearing welding helmets that had their interior components in defective and poor conditions. Employees were thereby potentially exposed to radiation hazards. The defective and poor conditions found in the personal protective equipment were the following;   1. The NeXGen (Jackson - Safety) gray welding helmet had its plastic head band piece cut in two and these two pieces were held together with gray utility tape.   2. The CSA (Z 94.3) black welding helmet had its cushioned (sweatOpad) head rest piece broken, cut, and a piece of it was missing.
