29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  Ervin Burkholder DBA Countryside Builders on or about April 07, 2014, at the address of 6539 S. Dixie Hwy Cygnet, OH: The employer did not ensure employees were adequately protected from falls while performing wood framing activities atop a residential structure.  During this work, the employees were exposed to falls of approximately 10 ft. to the ground below.

Dixie Hwy Cygnet, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing various wood framing operations from elevated surfaces.
