29 CFR 1910.132(d)(1)(i): When the employer had assessed the workplace hazards and determined that hazards were present, the employer did not select and/or use the types of personal protective equipment that would protect the affected employee from the hazards identified in the hazard assessment:   On or about April 8, 2014, an employee was exposed to a fall hazard when working from the basket of an aerial lift approximately 30 feet above a lower level while wearing a body belt with a 6-foot shock absorbing lanyard attached to the basket of that lift.
