On April 9, 2014, the employees were not protected from electrical conductors where the face plate on electrical outlets where missing on the East wall of the break room, exposing employees to electrical hazards. c. On April 9, 2014, the employees were not protected from an electrical outlet box that did not have the correct cover plate installed in the West wall of the break room, exposing employees to electrical hazards.
