29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:        a) Front roof: Employees without any means of fall protection were engaged in roof work from a pump jack scaffold platform, being exposed to a fall of approximately 19 feet onto a concrete sidewalk, on or about 4/1/2014.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder:     a) Roof right side: Employees were accessing the scaffold platform with an ladder that did not extend 3 feet above the platform, exposing employees to a fall of approximately 19 feet, on or about 4/1/2014.
