29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end:       (a) Jobsite:  On or about April 16, 2014, an employee was working from a pump jack scaffold and the one end extended about 4 1/2 feet over its end support.  In addition, this pic was not secured at either support bracket to prevent cantilevering when the employee worked beyond the pump jack pole..

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:     (a) Jobsite:  On or about April 16, 2014, an employee was working from a pump jack scaffold and he was always protected from falling by a standard guardrail system.  The fall  hazard to the employee was about 15 1/2 feet to the ground.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:     (a) Jobsite:  On or about April 16, 2014, an employee was using a 20 foot extension ladder and the top of the ladder only extended about 1 foot above the pic to the pump jack scaffolding.  There was no hand hold available for the employee.  In addition, this ladder was not secured at the top.
